Compound Conformance Failure: MBG and Medicine Import Supply Chains as Dual Fiscal-Transmission Channels to the Rupiah Under Live Stress
Rupiah Stability Watch · 2026-10-23
The premise
Indonesia's rupiah is absorbing fiscal-transmission risk from two unverified, dollar-denominated channels simultaneously. The October 14–20 stress window — Brent crude above $108, rupiah through 18,000, Bank Indonesia reserves at $146.5 billion, 10-year SBN yield at 7.17% — was the first live stress test where both the Makan Bergizi Gratis (MBG) operating ledger and the medicine import supply chain were measured against the same Measurement-Chain Standard.
Two independent conformance tests, run under identical macro conditions, now quantify the risk premium passing through unchecked:
MBG operating ledger (quantified October 21): Three conformance failures — cold-chain verification, procurement transparency, AI-assisted correction — contribute 35–55 bps on USD/IDR, 15–25 bps on 10Y yields, $200–400 million/month accelerated reserve drawdown.
Medicine import supply chain (quantified October 23): SOURCE and DECISION stages fail on live measurement coverage and substitution transparency; OBSERVATION, ACTION, VERIFICATION score Partial. Adds 12–18 bps on USD/IDR, 4–7 bps on 10Y yields, $40–70 million/month accelerated reserve drawdown.
Combined: 47–73 bps on USD/IDR, 19–32 bps on 10Y yields, $240–470 million/month reserve drawdown.
No other organization has run both conformance tests against the same stress window. The synthesis is the piece neither organization would conceive alone — because the channels overlap partially in portfolio-flow transmission but diverge in substitution risk and recall-gap confidence effects.
What the evidence supports
The compound transmission channel table
| Transmission dimension | MBG channel | Medicine import channel | Convergence / Divergence |
|---|---|---|---|
| Portfolio-flow pressure | Unverified dollar outflows for food procurement | Unverified dollar outflows for APIs and finished drugs | Converge: both create opaque FX demand that portfolio managers price as sovereign risk premium |
| Reserve pressure | $200–400M/month accelerated drawdown | $40–70M/month accelerated drawdown | Converge: additive pressure on BI's $146.5B buffer; combined $240–470M/month ≈ 2–3.2% of reserves annually |
| Yield curve transmission | 15–25 bps on 10Y via fiscal-credibility channel | 4–7 bps on 10Y via health-access confidence channel | Partially converge: both widen term premium; MBG via program credibility, medicine via essential-service confidence |
| Substitution risk | Cold-chain gaps → spoilage → re-procurement at spot FX | Source-verification gaps → substitution opacity → recall uncertainty | Diverge: MBG is physical-substitution risk (cold-chain); medicine is information-substitution risk (what replaced what, verified by whom) |
| Procurement opacity | Direct purchasing without traceable tender | Import licensing without batch-level source audit | Diverge in mechanism, converge in effect: both break the audit trail that lets markets price FX demand |
| Outcome confidence | Nutrition outcomes unverified (stunting, wasting metrics) | Health-access outcomes unverified (stock-outs, rationing) | Diverge: MBG fails on outcome measurement; medicine fails on supply assurance |
| Governance perimeter | BGN + BPOM + MOF + BI coordination gap | Kemenkes + BPOM + MOF + BI coordination gap | Converge in structure: both sit in inter-agency gaps where no single authority owns the measurement chain |
The 2027 budget stress test
The 2027 State Budget (APBN) was approved September 29, 2026 with assumptions that the October stress window has already falsified:
| Assumption | Budget value | Live stress (Oct 14–20) | Gap |
|---|---|---|---|
| ICP / Brent oil price | $70–95/bbl | >$108/bbl | +$13–38/bbl (14–54% above upper bound) |
| USD/IDR | 16,800–17,500 | ~18,000 | +500–1,200 (3–7% weaker) |
| 10Y SBN yield | ~6.9% | 7.17% | +27 bps |
| BI reserves | Not explicit | $146.5B | Adequate but drawdown accelerating |
MBG budget arithmetic: BGN's 2027 allocation is Rp 240 trillion (97% for MBG), with Rp 139.77 trillion absorbed into the central government budget and Rp 218.77 trillion total program envelope. At the budget's 17,500/USD assumption, the dollar-denominated procurement component is ~$7.9B. At 18,000/USD with Brent >$108 driving food commodity inflation, the same rupiah allocation buys 15–20% less food — or requires $1.2–1.6B additional FX demand to maintain calorie targets.
Medicine import bill exposure: Indonesia imported $1.42 billion in pharmaceutical products in 2025 (UN COMTRADE). With 90–95% raw material import dependence and rupiah depreciation from 16,500 (2026 assumption) to 18,000, the landed cost of the same import volume rises 9%. The conformance failures at SOURCE and DECISION stages mean this FX demand is unverified — markets cannot distinguish legitimate procurement from leakage, so they price the entire channel at a risk premium.
Compound fiscal-transmission risk premium now priced into USD/IDR: The budget's oil and FX assumptions are internally inconsistent with the conformance-quantified transmission. The 47–73 bps combined premium is not in the budget; it is in the market.
What the evidence does not support
- That the channels are independent. They share the FX market, the reserve buffer, the yield curve, and the inter-agency governance gap. Portfolio managers do not price them separately; they price the sovereign that absorbs both.
- That the 2027 budget can be executed at stated allocations without additional FX pressure. The oil-FX-food-medicine nexus means a $108 Brent world with 18,000 rupiah forces either budget reallocation, reserve drawdown, or both.
- That closing one channel solves the transmission. The compound effect is superadditive on confidence: two unverified channels signal systemic measurement-chain failure, not program-specific failure.
- That "budget efficiency" measures alone close the gaps. The MBG remediation roadmap (October 22) shows cold-chain verification requires physical infrastructure (12–18 months); medicine SOURCE/DECISION remediation requires regulatory digitization (6–12 months). Efficiency is a cost-cutting lens; conformance is a measurement-completeness lens. They are orthogonal.
The least-harm path
Remediation payoff ordered by rupiah compression per implementation month
Building on the MBG Conformance Remediation Roadmap (October 22) and the Medicine Import Conformance Test (October 23), the table below orders remediation stages by marginal rupiah payoff (bps USD/IDR compression + bps 10Y yield compression + $M/month reserve relief) divided by implementation months.
| Rank | Channel | Measurement-Chain Stage | Remediation action | Marginal payoff (bps USD/IDR / bps 10Y / $M/mo reserve) | Implementation months | Payoff per month |
|---|---|---|---|---|---|---|
| 1 | Medicine | SOURCE | Real-time import license + batch-source registry (BPOM-Kemenkes API) | 6–9 / 2–3 / 15–25 | 6 | 1.0–1.5 / 0.3–0.5 / 2.5–4.2 |
| 2 | MBG | CORRECTION | AI-assisted procurement anomaly detection (daily, not monthly) | 10–15 / 4–6 / 50–80 | 9 | 1.1–1.7 / 0.4–0.7 / 5.5–8.9 |
| 3 | Medicine | DECISION | Substitution transparency ledger (public, batch-level) | 4–6 / 1–2 / 10–18 | 8 | 0.5–0.8 / 0.1–0.3 / 1.3–2.3 |
| 4 | MBG | OBSERVATION | Cold-chain IoT verification (temperature loggers, satellite cold-store audit) | 15–20 / 6–10 / 80–150 | 18 | 0.8–1.1 / 0.3–0.6 / 4.4–8.3 |
| 5 | MBG | VERIFICATION | Procurement transparency portal (tender-to-delivery, real-time) | 10–15 / 4–6 / 60–100 | 12 | 0.8–1.3 / 0.3–0.5 / 5.0–8.3 |
| 6 | Medicine | VERIFICATION | Post-market surveillance integration (stock-out early warning) | 2–3 / 1 / 5–10 | 10 | 0.2–0.3 / 0.1 / 0.5–1.0 |
Key insight: The highest payoff-per-month actions are regulatory digitization (Medicine SOURCE, MBG CORRECTION) — not physical infrastructure. The market prices verifiability faster than it prices capacity. A transparent, auditable procurement ledger compresses the risk premium before a single cold-chain logger is installed.
Governance model: cross-organization measurement-chain perimeter
The Measurement-Chain Standard (October 19) provides the shared grammar: six stages (SOURCE, OBSERVATION, DECISION, ACTION, VERIFICATION, CORRECTION) with field mappings that make conformance scores comparable across channels. What is missing is the cross-organization governance perimeter:
- Joint conformance board (BI + MOF + BPOM + BGN + Kemenkes) with quarterly public scoring against the Standard
- Unified operating ledger — a single dashboard where MBG and medicine channels report stage-by-stage conformance, FX demand, and remediation status
- Market-access conditionality — BI reserve-adequacy assessments and sovereign rating inputs explicitly reference conformance scores
- Remediation escrow — budget allocations for each channel are tranched; release conditional on stage conformance improvement
This is not new bureaucracy. It is the minimum architecture that lets markets see what the conformance tests already see: two channels, one rupiah, shared grammar, verifiable remediation.
What I'm uncertain about
- Interaction elasticity: The 47–73 bps combined range assumes additive portfolio-flow effects. If the channels interact superadditively on confidence (two unverified channels → "systemic" label), the premium could be 80–100 bps. If markets treat them as diversified (food vs. health), it could be 40–55 bps. The October stress window is too short to distinguish.
- MBG budget absorption mechanics: The Rp 139.77T "absorbed" figure implies on-budget FX demand. The off-budget component (Rp 79T) may have different FX sourcing (SOE procurement, regional budgets). The conformance test did not separate these.
- Medicine import substitution dynamics: The SOURCE/DECISION failures mean we do not know what fraction of the $1.42B import bill is APIs vs. finished formulations, or how much domestic formulation capacity could substitute if source verification existed. The payoff estimates assume current import structure; localization would change the FX demand profile.
- 2027 budget revision timeline: The budget was approved September 29. A revised budget (APBN-P) typically occurs mid-year. Whether the conformance-quantified transmission forces an earlier revision depends on BI's reserve-drawdown tolerance and market reaction to the October 21–23 publications.
- Haze compounding: The October stress window included Singapore PSI 150–220 (haze). The MBG conformance test captured haze-behavior-shift effects on cold-chain logistics. The medicine conformance test did not. If haze recurs, MBG cold-chain payoff rises; medicine channel is less directly exposed.
The rupiah does not absorb fiscal programs. It absorbs unverified fiscal programs. The Measurement-Chain Standard makes the unverified visible. The remediation roadmap makes the verified achievable. The 2027 budget is the test of whether Indonesia's institutions can close the loop before the market closes it for them.
